We are very excited to announce that the Cardinal Stritch Catholic High School & Academy Athletic Department has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students.

 

The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your children, saving you time. We are requiring that ALL parents and all athletes for grades K-12 use Final Forms. You may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any update. Click on the Parent Playbook to get started in FinalForms.

 

HOW DO I SIGN UP?
Please register at: https://cardinalstritch-oh.finalforms.com/ and follow the prompts to create your account, create your students account and sign your forms. We are requiring that ALL parents and also ALL athletes for grades K-12 from this point forward to use FinalForms. You may review your data at any time to verify it is current. DO I HAVE TO SIGN UP WITH FINALFORMS IF I ALREADY TURNED IN MY PAPERWORK FOR A FALL SPORT?
Yes! You will need to sign up with FinalForms. This MUST be done before your child will be placed on a team. Failure to do so may result in your child being unable to participate.

 

WHY DOES MY CHILD NEED TO CREATE AN ACCOUNT, HE/SHE IS A MINOR?
All forms require a student signature therefore they need to create their own account, regardless of their age. Parents will need to assist younger children in this process as they may not be able to read/comprehend the information. It is important that they understand the need to tell their coach if they fall or are having chest pains etc… Please review the information and help explain it to your children and help them sign the necessary forms as a student athlete.

 

OHSAA requires that student athletes, grades 7-12, have a physical Evaluation Form completed by a physician.  See printable links below. 

 

WHAT IF MY CHILDS’ PHYSICAL ISN’T DUE YET?
Get the process started and create your account. Complete all forms except the physical. The Athletic Department will enter your child’s physical date and when it is due, you will receive email reminders, then follow the process when you get the physical completed. When you get your physical done, you will be required to upload the form – do not turn in a paper copy. Please make sure the CYO Athletic Contract (grades K-6) is signed by both a parent and a student.
